Complaint Submission

We are collecting information regarding medical transportation. We recommend you submit the complaint to ModivCare or Medical Answering Services (MAS) depending on your county and the Department of Health at medtrans@health.ny.gov or 518-473-2160. Official Complaint Process Information is found on the Department of Health Website

We are asking you to provide additional information to our project so we can analyze medical transportation complaints to ensure they are being recorded and reported correctly. Thank you for your assistance.
